PAYMENT INFORMATION
For most patients and most procedures, the care provided by the Urologists are covered by provincial health care. On occasional, there are exceptions and additional fees are charged.
These fees are to be paid to the office staff, by way of cash or cheque only.
As the Urologists are not in the office every day, please anticipate that this request can take from 7-10 business days. If there are specific forms for completion, please provide ASAP to avoid further delay. If a letter is required, please advise as to whether specific information is to be provided such as date of surgery, how many days you will be off work, limitations to return to work, etc. Once the paperwork is ready, the office staff will contact you so please make sure your phone number is current in our system.
There is a charge for this service and the amount is dependent on the time involved by the Urologist.
Do I need to book an appointment to get a doctor's note or get paperwork completed?
If you are a current patient, and have been seen within the past 12 months, you do not need an appointment. Please contact the office staff, who would be happy to discuss this with you.

HOW TO PAY FOR ELECTIVE PROCEDURES?
Elective procedures are not covered by OHIP. They may be covered by your third party health insurance plan or can be claimed on your taxes. However, Guelph Urology does not take responsibility for reimbursement advice.
The payment for elective procedures must be done in full, to the office, prior to the surgery date.
Payment is by cash or cheque, and the office staff can discuss this further.
Prescription renewals
If paperwork is required for special approval of prescription medication, please allow 5 business days for completion.
There is a charge for this service
We do not fax prescription renewals. If you require a new prescription, please call the office to make a follow-up appointment
If an office appointment is missed or cancelled with less than 24 hours notice, a cancellation fee will be charged. An invoice will be mailed out and a new appointment will not be booked until the invoice is paid in full.
If a surgery is missed or cancelled with less than 2 weeks notice, a cancellation fee will be charged. An invoice will be mailed out and a new surgery date will not be booked until the invoice is paid in full.
